How a throat damper works and where it sits
A throat damper is a hinged metal plate in a frame set into the masonry just above the firebox. Open, it gives smoke the full passage into the flue, and closed, it stops the flue drawing air from the room between fires. Section R1001.7.1 of the residential code calls for a ferrous metal damper at least 8 inches above the top of the opening, worked from inside the room.
The plate turns on pivots and is worked by a poker handle, a lever or a rotary crank through the fireplace face. Each part can fail on its own, which is why chimney damper repair starts by finding the one that did.
Why dampers stick, warp and leak
Rain entering an uncapped flue scales the seating edge until the plate will not close or will not move. A thin plate held over a hot fire can warp and stop seating. Pivots rust or wear loose, rotary controls wear at the gear, and fallen mortar or nesting material can wedge the plate.
Section R1001.6 requires the passage above the firebox, including the throat and damper, to be no smaller than the flue, so a plate stuck at half travel is a repair item even when the fireplace seems to draw.
A clamp holding the plate partly open is not always a fault. A vented decorative gas appliance listed to ANSI Z21.60 requires the damper fixed open to a minimum area, and that clamp stays.
Replacing the plate or the whole damper
Throat damper replacement uses a matching plate when the frame is sound and a plate is still made. When the frame has rusted into the masonry, it is cut out and fireplace damper installation follows, with a new ferrous damper set at the required height and the throat rebuilt in refractory materials that cure before the first fire.
Where a frame cannot come out without major masonry work, a top sealing damper closes the flue from the top instead. The yearly inspection NFPA 211 sets for the chimney, fireplace and vent system includes the damper, so fireplace damper repair of any size gets a second look then.
