Open joints between flue tiles
Clay flue tiles are set end to end in refractory mortar, and the code wants those joints tight and smooth inside (FBC R1003.12). Over time the mortar erodes and a gap opens. Through it, heat reaches the brick behind the tile, and flue gases and condensate soak into the masonry.
Gaps show as dark rings around the tile, or as missing mortar when the flue is viewed with a strong light. A single open joint near the top is a different finding from open joints along the whole flue, and the finding states which one this is. Joints just above the smoke chamber get extra attention, since they meet the hottest gases.
The thimble in the chimney wall
A thimble is the sleeve a connector passes through to enter the chimney from the side. The code wants it made of fireclay, rigid refractory or metal, and fitted so the connector cannot pull out of the inlet or reach past the inner face of the liner (FBC R1003.16).
The inspection checks that the thimble is intact, tight in the masonry and at the right height for the stove. Cracked or crumbling thimbles, or a connector poking into the flue where it catches deposit, are noted, along with how close the thimble sits to any wood framing. An open thimble with no connector in it is noted too, since it lets room air into the flue.
The chase top on a factory-built chimney
Some factory-built chimneys rise through a framed chase finished in siding, with a metal cover on top. That cover sheds water off the chase and seals around the chimney pipe with a collar and a storm collar.
The inspection checks the cover for rust, dents that hold standing water, and a collar gap that lets rain run down the outside of the pipe. Inside, stains on the chimney sections or the framing below point to a cover that has already let water through. The chimney's own cap, part of the listed system, is checked at the same time. A cover that pools water tends to rust through at the low spot first.

Will you need to get into the attic?
If the chimney passes through an attic, yes, because the attic is where clearances to framing and fireblocking can be seen. If there is no safe access, the findings say what could not be checked.
