A damper plate that has rusted thin
A throat damper is a cast iron or steel plate on a pivot, sitting in a frame at the top of the firebox. Rust works on it from both sides, with moisture coming down the flue from above and ash and condensation below.
Once the plate thins, it warps, stops seating in its frame and eventually breaks at the pivot. We check the plate by feel and by tapping, since a coat of soot can hide how much metal is left.
