Some chambers are parged smooth; others step inward brick by brick. Either kind is checked for gaps, loose or missing brick, and mortar that has fallen away, since heat and gases escape through those openings into the chimney walls.
A stepped chamber also holds deposit on every ledge. When the surface has failed, the finding points to smoke chamber repair as a separate job and says whether the fireplace can be used in the meantime.
Flue area against an appliance connector
For a stove, furnace or other appliance, the flue it vents into may not be smaller in area than the connector feeding it (FBC R1003.14). The inspection reads the connector size and compares it with the flue or liner. Too small restricts the exhaust; much too large lets it slow and cool. For gas appliances, sizing comes from NFPA 54 and the appliance's installation manual instead.

The chase top on a factory-built chimney
Some factory-built chimneys rise through a framed chase finished in siding, with a metal cover on top. That cover sheds water off the chase and seals around the chimney pipe with a collar and a storm collar.
The inspection checks the cover for rust, dents that hold standing water, and a collar gap that lets rain run down the outside of the pipe. Inside, stains on the chimney sections or the framing below point to a cover that has already let water through. The chimney's own cap, part of the listed system, is checked at the same time. A cover that pools water tends to rust through at the low spot first.
