Reading a crack in a refractory panel
Refractory panels in a factory-built fireplace are cast pieces held in a steel firebox, and fine surface cracking from heat is expected in them. The maker's manual draws the line, by crack width, by gaps opening between pieces, or by any piece that has loosened or fallen away.
Once a panel has broken through and the steel behind it is exposed to the fire, the unit should not be used until the panel is replaced. We measure what is there against the manual. Each panel is listed by its position and the maker's part number, and panels are replaced as a set when the manual calls for it.
When the house pulls against the fireplace
A fire needs air, and in a tight house with a range hood, bath fans or a clothes dryer running, the fireplace can become the easiest path for outside air to come in. Smoke then comes down instead of going up. The code's answer is an exterior air supply to the fireplace, unless mechanical ventilation keeps the room at neutral or positive pressure.
On a repair visit we test it directly: a fire lit with every exhaust fan off, then with them on, and then with a window open. The difference between the three tells us whether the fireplace or the house is the problem.

A dark fan of soot on the brick above the opening is a record of smoke spilling out of the fireplace. Cleaning it off is easy, but the stain comes back unless the cause is found. The shape helps. Staining across the full width of the opening points to an opening too tall for its draft, while staining on one side can point to a damper that opens unevenly or a fire built off center. We read the stain, run a test fire to confirm, and the fix for the cause comes with cleaning the face with a cleaner that suits the brick or stone.
