The shape of a new smoke chamber
The smoke chamber sits between the damper and the flue, and the code limits its shape. Its inside height, from the throat to the start of the flue, is no greater than the inside width of the fireplace opening. Its walls lean no more than 45 degrees from vertical when built with prefabricated linings or rolled walls, or 30 degrees when corbeled. They are at least 8 inches thick, or 6 inches with a firebrick or clay lining, and the inside is parged smooth with refractory mortar.
A chamber built to those limits carries smoke up cleanly. It is the part of a masonry build that is hardest to fix later.
Old chimney pipe, new fireplace
Replacing a factory-built fireplace raises one question before any other: can the existing chimney stay? The answer is in the new unit's manual. Some makers list chimney systems that match their older models, and some list only their current pipe.
If the manual does not name the existing pipe, it comes out with the old unit, including the firestops and the cap, and the new system goes in complete. We do not pair a new fireplace with pipe it was not tested with. The manual's page on chimney components is the document we read for this, and we cite it.
What can cover the face of the unit
The face of a factory-built fireplace, the metal around the opening, gets hot, and the manual limits what can cover it. It calls for noncombustible materials such as tile, stone or brick veneer, set with an adhesive rated for the temperature the manual names, and it shows where the facing has to stop short of the opening or the grilles. Combustible board and wood trim stay outside that zone.

How much floor space does the hearth take?
For a masonry fireplace, the extension reaches 16 inches out from the opening, with 8 more on each side, and grows for openings of 6 square feet or larger. A factory-built unit follows its listing. The plan draws it to size.
Can a new fireplace go on an upper floor?
Yes for a factory-built unit, because it sits on a framed floor and its chimney runs up from there, as long as the floor framing is sized for the load. A masonry fireplace needs a footing at the ground, so it has to rise from the lowest level.
