The thimble in the chimney wall
A thimble is the sleeve a connector passes through to enter the chimney from the side. The code wants it made of fireclay, rigid refractory or metal, and fitted so the connector cannot pull out of the inlet or reach past the inner face of the liner (FBC R1003.16).
The inspection checks that the thimble is intact, tight in the masonry and at the right height for the stove. Cracked or crumbling thimbles, or a connector poking into the flue where it catches deposit, are noted, along with how close the thimble sits to any wood framing. An open thimble with no connector in it is noted too, since it lets room air into the flue.
Flue area against an appliance connector
For a stove, furnace or other appliance, the flue it vents into may not be smaller in area than the connector feeding it (FBC R1003.14). The inspection reads the connector size and compares it with the flue or liner. Too small restricts the exhaust; much too large lets it slow and cool. For gas appliances, sizing comes from NFPA 54 and the appliance's installation manual instead.
When two appliances share one flue, each connection is checked, since the combination changes what the flue has to carry. The manual for each appliance is asked for at booking so the check has the right numbers.

NFPA 211 has the yearly inspection take in every flue in the chimney and every appliance connected to it, including ones rarely used. A second flue that once served a furnace or a closed-off fireplace can hold nests, fallen tile or water damage with no one watching.
The code also wants a masonry divider, a wythe of 4 inches or more, between flues in the same chimney (FBC R1003.13), and a missing or crumbled wythe lets smoke cross from one flue into another. Each flue is recorded by what it serves, or that it serves nothing. Unused flues get their caps checked too.
