A tile cracked through, or a joint between tiles that has opened, lets hot gases and sparks reach the brick shell behind it, and through that shell any wood framing set too close.

When an addition leaves the chimney too short
Chimney height is not settled once and forgotten. Section R1003.9 of the residential code measures the top of the stack against its surroundings: it has to clear anything built within 10 feet of it by 2 feet or more, and rise 3 feet or more above its own roof penetration.
A second story, a raised roof or a new dormer built near an existing chimney can leave it short of that line, and the result is weak draft and smoke in the room when wind crosses the new roof. The fix for a short chimney is a design question: what it would take, and which trade would carry it out.

A steel angle spans the top of the fireplace opening and carries the brick above it. When water comes down the chimney, the lintel is one of the places it lands, and steel grows thicker as it rusts and scales. That growth can push the brick above it up and out, opening a crack along the top of the opening or tilting the face.
A lintel with surface rust can be cleaned and painted with a heat-rated coating. One that has lost thickness, or has already cracked the brick, needs replacing, which means supporting the masonry above while the old steel comes out.
