A hearth slab that has dropped

A hearth that has cracked straight across, with one side sitting lower than the other, has lost support underneath. The code calls for masonry hearths to be supported by noncombustible materials and reinforced to carry their own weight and the loads on them, and a slab set on wood framing or without enough reinforcement can settle. The repair starts below the surface. We check what the slab sits on before pricing anything, because resetting tile on a slab that is still moving produces the same crack again. When the support is the problem, the slab gets priced instead of the tile.

Raised hearths and the thinner extension

When the floor of the firebox sits at least 8 inches above the hearth extension, the code allows the extension to be thinner: at least 3/8 inch of approved noncombustible material instead of the 2 inches it otherwise requires. That exception is why a raised hearth can carry a thin stone or tile apron. It does not change how far the extension has to reach. A raised hearth that has been lowered, or a room where new flooring has built up the floor level, can take the fireplace out of the exception, so we measure the height as it stands today.

An opening too big for its flue

Flue size and opening size are tied together in the code: a square flue needs at least a tenth of the opening's area, a round one a twelfth, and a rectangular flue at least twice as long as it is wide needs an eighth. When the opening is too large for the flue, the draft cannot carry all the smoke, and some rolls out the top of the opening. We measure both and do the arithmetic on site. The fixes that work reduce the opening, with a metal smoke guard across the top or a raised firebox floor, each sized to bring the ratio back in line.
